Xcode 6.1 iOS 8 App Icon appears in the upper left corner of the first screen, cannot be moved or deleted

When I install my app on a device using xCode 6.1 and iOS 8, the app icon appears in the upper left corner of the first screen below the existing app icon and cannot be moved or removed, although I can tap it as it peeks from under another icon. Is this a known bug? How can this be fixed?
