Multi select custom adapter with search and checkbox

I am new to android app development.

I want to create a search and select functionality in ListActivity using CHOICE_MODE_MULTIPLE_MODAL. Below is the user interface. I want to create my list.

I've been stuck on this since two days who have a solution or a way to do this.
