What does the unknown version of the app mean in the play store

I got an unknown version from the play store. What does it mean? I'm confused. Increase the version code every time I download the app. Can anyone tell me what happened?

AND

The app icon gets a gray box also in the app store list. What does it mean? I am stopping two problems deploying some problems. Is this gray box the reason for stalled releases?
