What is a good PHP script I can use as a poll?

What is a free free PHP script to use as a survey for a website? I will need a simple script to show on a webpage with a standard multi-parameter chart + chart plot. You will also need an admin interface with adding / editing / deleting a survey.

I used the "Advanced Survey" and found it to be very good, and almost completely accurately described.
